Is there a deductible for dental coverage?
Why should I use a participating dentist? Using a participating dentist saves you money because participating dentists have agreed not to charge you for any amount over Delta Dental's allowable fee and they will file your claims for you. How do I know if my dentist is a participating dentist? Contact your dentist and ask if he or she participates in the Delta Dental Premier program or visit the Delta Dental website, select Delta Dental Premier Dentist Search, and enter the required data. If you would like to request a list of all participating providers in your area, contact a Delta Dental Customer Service Representative at 800-995-5120 (in St. Louis 314-656-3001). What if I choose to visit a dentist that is not in the Delta Dental Premier network of participating providers? You are free to visit the dentist of your choice to receive treatment. If you receive services from a non-participating dentist, you are responsible for paying the dentist up front and filing your own claim forms. The benefit payment is reimbursed directly to you. Benefit payments are based on the average fee charged by the majority of dentists in your area and you are responsible for any difference between the dentist's charge and Delta Dental's benefit payment. How do I get claims forms? Where do I submit completed claim forms? Here is a claim form (PDF). Delta Dental also accepts the standard (universal) ADA claim form that your dentist may use. *Submit Delta Dental Premier claim forms to: Delta Dental of Missouri, P.O. Benefit24, Delta Dental's 24 Hour Automated Information Retrieval System (PDF) at 800-995-5120 (in St. Louis 314-656-3001) is available to you or your dentist, toll-free, 24 hours a day, seven days a week. Follow the system prompts to receive detailed benefits information by phone or fax. Opt out to speak to a customer service representative Monday thru Friday from 7:00am to 5:00pm CST. How do I get a new I.D. card? Go to the Delta Dental web site , click on “Your Info,” “View General Benefits”, enter required information and select “Request an ID Card”. If you are in immediate need of an ID card, select “View Benefits” and you will be able to print an ID card. |
