We offer a free tobacco cessation program to employees and dependents (18 years of age or older) enrolled in the company medical plan. Enhance your health, your community and your financial well-being throughout 2012 by committing to 12 wellness habits. Whether you opt to volunteer, forgo plastic bags at the grocery store, boost your intake of fruits and vegetables or vow to pay down your debts, taking action now will put you one step closer to achieving your goals. This results in a large portion of the population knowing nothing about their biometric health numbers — things like blood pressure, blood sugar, body mass index and cholesterol levels. Healthy employees are our best asset and our best defense against rising health care costs. When visiting an in-network Choice Plus provider, preventive care services are covered at 100 percent of eligible expenses without deductible, coinsurance or office visit copay for both PPO300 and PPO600. When visiting a EPO Choice network provider, preventive care services are covered at 100 percent of eligible expenses without coinsurance or office visit copay. When your appointment is for preventive care, remind your physician to code such visits as "wellness" or "preventive" when he or she files your claim.
